申请材料
（1）：当前有效的护照（如有旧护照则一并提供原件）。
（2）：婚姻状况文件。已婚人员提供结婚证复印件。
（2）：一份打印出来且已经填写完成，并经本人签字的申请表。
（3）：一张护照照大小的彩色照片（45毫米高，35毫米宽）
（4）：现金支付相应签证申请费，已经在网上支付的，带上Worldpay发的确认函作为已付款的证明。
授权委托书（英文版）
